Advanced Passenger Information System Form

The US Government requires that airlines collect and transmit the following information. The Advanced Passenger Information System (APIS PLUS) will require more detailed data. The APIS procedure applies to all air transport passengers, entering, leaving or in transit in USA. All passengers, traveling on all flights are subject to these requirements and all pertinent information must be entered into their reservation.

In order to comply with these new requirements, we ask that you complete this form and submit it to our office. This information must be received before ticketing can be processed.

Non-US residents are required by US law to provide their full address including street name and house number, city, state and ZIP code as well as hotel name (if applicable).
